Evie Life Psychiatry is dedicated to providing accessible, compassionate, and evidence-based mental health care, specializing in psychiatric evaluation and medication management by licensed psychiatric nurse practitioners.
2. Core Services provided
Evie Life Psychiatry provides comprehensive outpatient psychiatric and behavioral health
services for patients ages 16 and older through both in-person and telepsychiatry appointments.
Core services include the following, but are not limited to:
- Comprehensive psychiatric evaluations
- Diagnostic assessments
- Medication management
- Substance use disorder screening and treatment support
- Individualized treatment planning
- Supportive psychotherapy
- Care coordination and referral services
- Crisis assessment and safety planning
- Telepsychiatry services
- Integrated treatment planning for co-occurring behavioral health conditions
The clinic utilizes evidence-based practices and individualized treatment approaches tailored to each patient’s clinical presentation, psychosocial needs, and treatment goals.
3. Provider Credentials
All care is delivered by board-certified psychiatric mental health nurse practitioners (PMHNP-BC) licensed in Illinois, practicing independently under full practice authority as allowed by Illinois Nurse Practice Act (20 ILCS 2105/).
4. Hours of Operation

- Monday – Friday: 9:00 AM – 5:00 PM

- Weekends and holidays: selected availability

- After-hours: Non-urgent messages may be left on our voicemail or patient portal and will be returned the next business day.
5. Appointments and Cancellation Policy

- Scheduling: Appointments must be scheduled through our secure portal, phone, or email.

- Cancellation: At least 48 hours’ notice required.

- No-Show Fee: $75 for missed appointments without prior cancellation.

- Multiple no-shows may result in termination of care.
6. Telehealth Policy (Illinois)
Under Illinois law (Public Act 101-0642), telehealth is fully authorized for behavioral health.

- You must be physically located in Illinois at the time of session.

- A reliable internet connection and private space are required.

- No telehealth is provided for controlled substance prescriptions for new patients unless allowed under DEA rules and state exemptions.
Billing & Payment

- We accept most major commercial insurances and Illinois medicaid and Medicare

- Payment is due at time of service unless otherwise arranged.

- A card on file is required for all patients.

- Patients are responsible for knowing their benefits and coverage.

- We accept a variety of insurance plans, including Medicaid, and offer a Sliding Fee Discount Program (SFDP) for uninsured or underinsured patients based on household income and family size.
7. Prescription & Refill Policy

- 48–72 hours required for refill requests.

- Controlled substances (e.g., stimulants, benzodiazepines) require periodic in-person or video visits and may be subject to Illinois PMP review.

- Refill requests must be submitted through the patient portal.
8. After-Hours Coverage
Evie Life Psychiatry ensures continuity of care through 24/7 access to clinical guidance.
Patients may contact the practice via phone after hours and receive instructions for urgent concerns. Emergency guidance includes contacting 911 or going to the nearest emergency department. Crisis resources, including the 988 Suicide & Crisis Lifeline, are routinely provided.
Urgent calls are returned by the provider, and all after-hours interactions are documented. Follow-up is conducted the next business day.
